Mara killed an orc follower then Mara split producing clones that are vengeances targets (Mara splitting before killing the orc doesn't seem to make the clones vengeance targets), the player then left the level and gained enough piety to return their orc to life ending vengeance and then re-entered the Mara level which because it had been long enough for the clones to timeout they timed out on entering the

If your orc follower dies with Pikel and band in view and then you bring Pikel but not his band to a different level and kill him and then gain enough piety to end vengeance, going back to the level that has Pikel's minions will kill them a daction before running the daction to clean up their vengeance target enchantments.
